Creative Assignment: Planning the photoshoot

Before I can do my photo shoot i have to thinks of all the things i may need to take thinks about the considerations of a photo shoot. To display this i have created a mind map of my  thoughts here :

For my photoshoot i will use the model Eloise Brayant. To use the photos taken i will need a model release form. This form covers the shoot and allows the photographer to use the photos in a public place. Here is my model release form :
